Integrated Family Support Team - Edge of Care Team

What we do

Pembrokeshire County Council’s Edge Of Care Team provide intensive evidence based intensive interventions to families where children and young people are on the edge of the care system and may become Children and Young People Looked After by the Local Authority. The interventions are often bespoke and innovative packages of support offered to families to work alongside them to address their presenting needs. The team aim to keep families together or enable children and young who become looked after to return home safely. The team work closely with families to understand the challenges that they face as individual family members and as a family unit. The aim to work alongside families to create suitable plans to increase safety and family wellbeing to enable families to remain together.

Where are we located?

We are located in County Hall, Haverfordwest and we cover the whole of Pembrokeshire.

How do you access our support?

To access support from the Edge of Care Team, a Pembrokeshire County Council children's services social worker would need to be involved with you and your family. You can call your social worker directly on their mobile or via their team and request them to make a referral on your behalf. If you don't have a social worker and feel that you need support, you can contact the child care assessment team on 01437 776444.
