WWe provide respite care, assessment & rehabilitation support for adults who need support, care or protection. We work in partnership with Social services & The Health Board. What does Havenhurst Care Home provide?

Havenhurst Care Home provides two types of services:

  • Respite Care - The Respite care service can be booked throughout the year enabling the person & their carers to have a planned break.
  • Rehabilitation - Rehabilitation service provides support to enable people to regain & maintain their independence as far as possible. There is a bungalow set in the grounds of Havenhurst which is used for this service.
  • Residential Care (long-term care) for individuals who need substantial help with their daily living needs.

Havenhurst Care Home provides the following:

  • Healthy living & well -being support
  • Assist with individualised care needs
  • Various activities 
  • Nutritional & varied meals with dietary needs catered for.
  • Laundry

How much will it cost?

There may be a charge for staying in Havenhurst Care Home. People are asked to complete a financial assessment in order to determine ability to pay & how much will be charged for respite services. People who stay for a period of time on assessment will not be expected to contribute financially.

Hillside Residential Home

Hillside Residential Home provides a residential care service for adults mainly over 65 years old who need support, care rehabilitation or safeguarding including people leaving hospital; working in partnership with Social Services & Hywel Dda.

Hillside Residential Home is open 24 hours a day throughout the year & has respite, intermediate care & permanent beds.

How to access Hillside Residential home?

A person can access Hillside Residential Home for Respite or Permanent beds following an assessment of need & referral from Adult Care.

To access Hillside Residential Home for the intermediate care service the person will need to have a joint social care & health assessment.

What does Hillside Residential Home provide?

Hillside Residential Home provides 3 types of services

  • Respite Care

The Respite care service can be booked throughout the year enabling the person & their carers to have a planned break.

  • Intermediate care

The intermediate care service provides rehabilitation therapy for up to 6 weeks to enable people to maintain their independence as far as possible. Where applicable this may be offered to people from 45years of age where the assessed need is identified.

  • Permanent Beds

The Permanent beds service provides a home for those in need.

  • Nutritional & varied meals with dietary needs catered for.
  • Laundry

We provide in house day services based in six day centres across Pembrokeshire.

The six day centres are:

  • Meadow Park ,  Haverfordwest
  • Havenhurst, Milford Haven
  • Wintern, Goodwick
  • The Anchorage SAC, Pembroke Dock
  • Bro Preseli, Crymych
  • Lee Davis, Narberth  


When are the Day Centres open? 

The service operates a programme of activities across the week in each locality. The service does not operate on designated staff training days and excludes bank holidays.  

 

What do the Day Centres Provide? 

The Day Centres provide a day service for older people, people diagnosed with dementia and adults who have a learning disability that live in the community. The day centre provides support to people who need extra assistance to enable them to maintain their independence.

The purpose of Day Centres is to provide a service that:

  • Values & respects people as individuals
  • Promotes well-being
  • Supports & enables people to maximise independence
  • Supports people to make decisions about the services they receive
  • Allows carers to have valuable time away from their caring responsibilities

The Day Centres provides the following opportunities:

  • Healthy living & well–being
  • Sports / Leisure & Gentle Exercise
  • IT and Digital
  • Lunchtime meal & refreshments
  • Arts & Crafts
  • Cooking & Baking
  • Community
  • Reminiscing
  • Nature / Gardening

Activities take place in our centres we use local community facilities such as leisure centres, local libraries and work with third sector organisations.  

 

How much will it cost? 

This depends on the outcome of a financial assessment in order to determine ability to pay. 

The day care charge for council owned Day Centres is £32.49 per day.

There is currently a maximum charge of £100 per week for all community-based services.  

Where lunches are provided by our in-house catering department they are charged at the current rate of £4.86 per day. Alternative provision is available at some sites and costs vary.

There is an additional charge for some activities.  

Transport 

People can use their own means of transport to access Day Centres or transport may be provided subject to eligibility & availability. This will be discussed in your assessment.

There is also the option of community transport that could be used to travel to a day centre Social Services Transport - Pembrokeshire County Council

Respite Care

Respite care is offered to our customers for a designated period of time (dependent on level of need) in an appropriate setting or via a direct payment. The aim is to provide a period of rest for the individual, and their carer, who may be finding it difficult to maintain their full independence at home, or in their care setting.

Respite is provided in various settings. These include:

  • Havenhurst / Milford House, Milford Haven - Mental Health & Elderly Mental Infirm customers
  • Residential Homes.

Milford House Centre

Milford House Centre provides services for customers with mental health problems who need support.

The centre includes:

  • Yr. Hafan - 24 hour supported accommodation consisting of 5 Rehabilitation beds
  • Haven View – 24 hour supported accommodation providing respite and assessment.
  • Yr. Nyth – A 3 bedded flat, where staff provide support through the Housing Support Grant based on assessed need on a short term basis.
  • Trafalgar Road Project - 6 single flats, were staff provide support through the Housing Support Grant based on assessed need on a short term basis.

 

Access to this service is through the mental health team assessment process.
