Integrated Family Support Team - Edge of Care Team
About the team
The Edge of Care Team is part of Pembrokeshire County Council's Integrated Family Support Service (IFSS). We work with children, young people and families who are experiencing challenges that may place children at risk of entering care. Our aim is to help children remain safely within their family networks wherever possible. We provide intensive, evidence-informed support tailored to the unique strengths, needs and circumstances of each family.
Working alongside families, we focus on building resilience, improving relationships, increasing safety and supporting long-term positive change. When a child or young person has entered care, we may also support plans for a safe and sustainable return home where this is in their best interests.
How we support families
Support provided by the Edge of Care Team may include:
- Intensive family support and intervention.
- Practical and emotional support for parents, carers, children and young people.
- Strengthening family relationships and communication.
- Supporting families to manage conflict and challenging situations.
- Connecting families with their wider family and community support.
- Developing safety plans and strategies to improve wellbeing.
- Supporting children and young people at risk of family breakdown or entering care with the aim to prevent this where it is safe to do so.
Our approach is strengths-based, collaborative and focused on achieving positive outcomes for children and their families.
Where we are based
The Edge of Care Team is based at County Hall in Haverfordwest and provides support to families across Pembrokeshire.
Accessing Our Support
Referrals to the Edge of Care Team are made by Children's Services Social Workers who are supporting families. If you are currently working with a Pembrokeshire Children's Services social worker, you can speak with them about whether support from the Edge of Care Team may be appropriate for your family.
If you are not currently receiving support from a social worker but are concerned about a child or feel your family may need support, please contact the Child Care Assessment Team on 01437 776444.
